Participating Insurance Plans

New Patients

The providers at PMA are accepting new patients for most insurance plans. Please have your insurance information available when calling to schedule an appointment and also make sure that you bring your identification card to your appointment.

Forms that will be needed for your first visit to our office are located on the FORMS page. Please download and complete the forms prior to your appointment and bring them with you.

Financial Policy

Payment is expected at the time of service for previous balances, copayments, co-insurance, and deductibles associated with your insurance plan. Your current insurance card is also required in order to bill for your visit. You may be required to pay for your visit in full at the time of service if we do not have the required information.

Patients are ultimately responsible for their accounts and will be required to pay for charges not covered by their insurance. Benefits cannot be guaranteed until your insurance company receives a claim for the services rendered. Please contact your insurance company directly if you have a question about what is covered under your policy.

Self-Pay Accounts

Piedmont Medical Associates understands that many patients may not have medical insurance coverage. We are able to provide a discounted rate to those patients providing the services are paid in full at the time they are rendered. A deposit may be required. Please contact our office for additional information about our self-pay policy.

Missed Appointment Policy

Providing advanced notice that you are unable to keep your appointment is an important courtesy that we ask of our patients. Please contact our office during regular business hours and as soon as possible if you are unable to keep your scheduled appointment. This will allow us to offer your appointment to another patient if necessary. Missed appointments without prior cancellation notice are subject to a charge of $25 for standard office visits, $40 for complete physicals and extended appointments.
